Registered Nurse Case Manager
You will have an individualized RN Case Manager who assesses and evaluates your in-home needs. The RN Case Manager will make a recommendation for a home care plan after the initial intake interview and in-home assessment. Your participation in the plan facilitates the best outcome. Your family is an integral element in helping to decide the best care plan. Our goal is to help you stay as independent as possible while keeping you safe in your home.
The RN case manager will select the level of health care provider appropriate for your needs. The case manager or designee is available 24 hours/day, 7 days/week for you and your family members to respond to emergent questions or needs. Different than many other agencies, we do not utilize an answering service. An experienced staff member is available to answer your calls 24 hours a day, 365 days a year.
Registered Nurse (RN)
Your Physician or RN Case Manager may delegate skilled nursing care to be given by a Registered Nurse. Coordination of services between physician and home care agency nurses may permit you to stay in your own home longer or expedite your return to home if you have been hospitalized. We have a staff of over 400 active RN’s with vast and diverse experience. Different from many other home health agencies, many of our nurses also currently practice in the hospital setting and are dual trained to provide both hospital and home care. Many of our nurses are also working in critical care units and heart care units in local hospitals. This dual advantage assures our nurses have current, state of the art knowledge and skills that they bring to the home setting. This unique circumstance offers our patients the highest level of clinical competency not always found in the home care setting.
Home Health Aide
A Home Health Aide is a companion for you in your own home offering services such as personal care (bathing & dressing), medication reminders, meal preparation & light cooking, light housekeeping, grocery shopping, laundry, and accompaniment to appointments.
Home health aides may be employed hourly, for specific care needs, or on a live-in basis. You can chose specific periods of time, days, evenings or nights, weekdays or weekends to employ our staff. You may interview the healthcare providers assigned to your care prior to placement if you desire. Our goal is to provide the highest level of care and satisfaction. The RN Case Manager delegates, supervises and evaluates the care delivered to you by the home health aide.
